Coordinated Care Conferences
Keeping every person on your child’s team aligned
Regular check-ins that bring together families, schools, and outside providers, so your child’s treatment plan is understood and reinforced by everyone involved — not just the clinicians at Integrity Care Co
What Are Coordinated Care Conferences?
The meeting that keeps your child’s plan from fragmenting
Children receiving EIDBI and ABA services often have several people involved in their care — teachers, school support staff, pediatricians, county case managers, and sometimes outside specialists. Without regular coordination, these providers can end up working from different information, or not communicating at all.
Coordinated Care Conferences are scheduled check-ins where our clinical team shares progress updates from your child’s Individual Treatment Program with everyone who needs them, and gathers input that helps refine the plan going forward.
For families, this means one less thing to manage. Instead of relaying updates between providers yourself, our team handles the coordination directly, while keeping you informed and involved in every decision.

Primary Care Physicians
Keeping your child’s doctor informed of therapy progress relevant to their overall care and development.
03

Coordinating with any providers outside Integrity Care Co so your child’s approaches stay consistent.
04
County Case Managers
Sharing updates required for continued EIDBI authorization and county-level service coordination.
05
Family Updates
Making sure you understand every update shared with outside providers, in plain, direct language.
06
Progress Documentation
Maintaining clear, current records that reflect where your child actually is in their treatment plan.
What a Conference Looks Like
Structured, useful, and easy to follow
We keep conferences focused, so they respect everyone’s time while still covering what matters.

01
Gather Progress Updates
Our team compiles current data from ABA, speech, and occupational therapy sessions before the meeting.
02
Share With the Right People
Updates are shared with the specific schools, providers, or case managers involved in your child’s care.
03
Discuss & Gather Input
We listen to feedback from teachers, physicians, or other providers that may shape the treatment plan.
04
Update the Plan
Any changes are reflected in your child’s Individual Treatment Program and communicated back to your family.
Preparing for a smooth transition to school
Coordination with schools naturally connects to our School Readiness program, which builds the routines and confidence children need for the classroom.
